Voluntary

Plans supported by the HealthShare/THA Insurance Program include: 

HealthShare Pharmacy Discount Card: The Card enhances a typical funded medical plan pharmacy benefit with greater discounts or supplemental coverage on medications not typically covered. It can also be used to save on over-the-counter products such as Nicorette and diabetic supplies. Employees and family members have unlimited use of the card, which is recognized at Walgreen’s, Tom Thumb, Target, Kroger, Randall’s, CVS and other major pharmacies as well as many independent.  To receive discounts on your prescripitons go to healthshareTHA.lc.healthtrans.com, download and print the pharmacy discount card.  No cost to issue and use the card.

    Interest Sensitive Whole Life Insurance Product:  The Boston Mutual interest sensitive whole life insurance product is a policy offering permanent protection with guaranteed premiums, cash values and death benefits.  The policy has guaranteed level premiums payable to age 95.  This policy is available with optional riders for waiver of premium, accidental death benefits, disability income, children’s insurance, insured level term to age 65, accelerated benefit option, and security plus/critical illness accelerated benefits option.

    Voluntary Disability Product:  The Boston Mutual Worksite disability product, the Employee Disability Option (“>EDO”), is a voluntary group disability product available only to the actively at work employees of client accounts.  It features elimination periods of 0 days to 2 years, and benefit periods from 30 days up to 5 years.  Coverage is restricted to non-occupational disabilities.  This product is underwritten by American Fidelity Assurance Corporation.

    Universal Life:  The ING Employee Benefits’ universal life insurance product is a cost-effective method of providing permanent life insurance during a person’s working years as well as during post-retirement.  Premium payments can be modified to accomplish personal financial objectives and cash values are credited with current interest rates. 

     

    Accident:  ING Employee Benefits’ accident insurance provides additional cash payments to cover the costs and expenses resulting from an accident.  Payments are made directly to the policy owner and can be used to offset hospital and doctor costs, lost time at work, co-payments, and deductibles.

    Critical Illness:  Certain illnesses can be diagnosed unexpectedly and may create an unforeseen financial impact for employees and their families.  ING Employee Benefits’ critical illness coverage pays an immediate, one-time, lump-sum (ranging from $5,000 to $100,000) benefit following the diagnosis of one of several specified diseases
    . 

     

    Scheduled Benefit Cancer:  Loyal American Life Insurance Company’s scheduled benefit cancer insurance provides benefits for daily hospital confinement and related expenses for treatment of cancer, including radiation and chemotherapy.  Optional riders provide first occurrence benefits, specified diseases and intensive care benefits.  Individual and family coverage is available.  Pre-existing conditions have been waived for takeovers of prior plans.

    Disability Income:  Loyal American Life Insurance Company’s disability income insurance provides monthly benefits for disability due to injury or sickness occurring on or off the job.  The maximum benefit is 60% of income not to exceed $5,000.  There is a benefit period of up to two (2) years with elimination periods of 14 to 30 days.  An optional accidental death benefit rider is available and the coverage is portable.  For ease of explanation and administration, occupational rates have been blended so that the premiums are identical for all employees regardless of job function. 

    GAP Plan:  Loyal American Life Insurance Company’s GAP plan insurance provides coverage from $1,000 to $10,000 for expenses resulting from an accident.  Optional deductible choices are available.  Optional riders offer hospital admission for sickness, injury or complications of pregnancy, daily hospital room benefit, intensive care confinement, outpatient surgery, critical illness, heart attack and stroke, cancer, accidental death and dismemberment and return of premium.  Individual and family coverage is available.
